  • Boxed warning for estrogens: increased risk of endometrial cancer (unopposed estrogen), stroke, blood clots, and, with progestin, breast cancer and probable dementia in women over 65. Use the lowest effective dose for the shortest duration.
  • Do not use with unexplained vaginal bleeding, history of breast or estrogen-dependent cancer, blood clots, stroke or heart attack, liver disease, or pregnancy.
  • Non-hormonal options (paroxetine, desvenlafaxine): possible nausea, sleep changes, sexual side effects; do not stop abruptly. Antidepressants carry a boxed warning for suicidal thoughts in young adults.
  • Some options are FDA-approved products; any compounded option is not FDA-approved.
